Purpose
This template provides a convenient way to add file information, the creator and source of the file, and the copyright holder.
Usage
The template has three main parameters:

|1=(file information)
|2=(attribution, if it is a fair use image found somewhere online)
|3=(source, if it is public domain or uses some other free license)
|4=(who owns the copyright)
}}
You can also customize the background and text color of the heading and the left cell on each row:
